While I very much enjoy cleaning up the blood and gore in the manner VCD currently allows me to, I'd like to see almost a challenge mode with mechanics similar to the game Papers, Please. The mode would be a progression of cleaning areas in a certain time limit such as a standard 8 hour work day (Sped up of course). You'd start a level, clock in, then clean as much as you can in the 8 hour work day, your progress would be tracked by some manner of progress bar either in the HUD or in the start menu, once the 8 hours is finished you must punch out because your company offers no overtime, if you don't punch out on time you receive a penalty. After punching out you would receive an amount of money based on the amount cleaned that day. You'd then have to pay expenses such as housing utilities, food for yourself and your family, and any other expenses that would occur. If you have any money left over maybe you could purchase a new or upgraded cleaning appliance. You'd have to be careful though, because the next day you might not do so well and you won't have enough money for food because you bought that fancy mop, you cocky bastard. The mode would either progress as a survival game where you see how far you can get before your family dies, or you get fired for poor performance, or you injure yourself on the job slipping in some blood and brain matter, or it could progress in a linear manner with set events occurring in the form of messages from your bosses. Just an idea to provide a mode with slightly more satisfaction and progression as opposed to how the game currently operates.

It sounds like a fun idea however the part where you pay for expenses such as your family and food i think is a bit too much. It would probably fit the game better if you could use the money you made to buy better janitor equipment such as a longer mop to reach high places or a brush and dustpan to pick up multiple small items at once and throw them into the incinerator.

The rest of your idea is something i would love to see in the game as the game needs a little more pressure at times.
